Vampire Facelift

Dr. Prasad: Today, we’re doing a Vampire Facelift® and a brief explanation of what a Vampire Facelift® is, basically the idea of lifting the face is what leads to some confusion. As a cosmetic surgeon, I perform face lifting routinely. And what I’m doing? I’m routinely vertically lifting the face. That means lifting the skin upwards. Now instead of lifting upward, the Vampire Facelift® concept is more about lifting the skin outward. Now why is that? Well, because as we get older, we lose bone, muscle and fat. And so the face starts to go inward. You start to see more of the skeleton, the underlying structure of the face.

Patient: Over the past few years, I’ve noticed that my skin doesn’t quite feel smooth and bright as it used to. I’ve noticed that there are certain small indentations that begun to appear under my eyes, in the areas around my nose and lips. And these are the kind of issues that I want to address but at the same time, without getting the very obvious artificial look. This is why I’m really looking forward and very excited for the Vampire Facelift®.

Dr. Prasad: So what the Vampire Facelift® is doing is we’re adding hyaluronic acid and platelet-rich plasma in a unique way to restore volume in areas that need volume. As you get older, as you hit your mid to later 30s and beyond, you start to lose areas of volume of fat particular such as the lateral part of your eyebrow, the outer part, the tear trough area or the under eye area, the cheek area, the nasolabial fold also called the parentheses line, the outer corner of the lip. So the concept of Vampire Facelift® is to literally to take one syringe of a hyaluronic acid filler such as Restylane, Juvederm, Belotero and use that very strategically. So that way we avoid an unnatural, over inflated effect and for people with these early stages of facial aging, they don’t need very much. The idea of using platelet-rich plasma is really very inspired. The platelet-rich plasma stimulates healing so it increases blood supply and collagen. Think about it when you get a cut, the area where you get the cut heals by generating collagen and new skin cells. So what we’re doing is we’re kind of manipulating that healing response and placing the PRP and stimulating new collagen and blood supply to the area that needs it.

Now the synergistic effect of hyaluronic acid and platelet-rich plasma has been very well documented in wound healing. And so this is a benefit that we get really nicely with the Vampire Facelift®. So our plan today is to add volume in the areas where she needs volume by doing a Vampire Facelift®. So we’re going to start first with a hyaluronic acid filler. So this happens to be Restylane but you can use Juvederm, Belotero or any filler that you are comfortable with. So what we’re going to do is first start with identifying where the opportunity is. Now to me, her lateral brow is already very nice and high so we’re not going to place anything in the lateral brow.

I think we have an opportunity right here in the tear trough area and hollows. She had already topical anesthetic and she should be relatively comfortable. I’m going to ask her to rest her head back so she doesn’t feel the pressure to stay steady and we’ll start placing. You can bring your chin up. So now we’ve got this area filled in very nicely. It looks very natural it’s blending in very well.

Hi my name is Dee, I’m 72 years old. I came here to see Dr. Prasad because I’m interested in having a facelift an eye job because of the fact that I don’t feel 72 years old. I’m very active. I’m still working. There are times at work where people look at me and say “Oh you look so tired and you haven’t slept.” But I do sleep, I have a good night sleep and I still look exhausted. I’m just became a grandmother at the age of 70. I love to keep with the little girl. They’re twins. When we’re out, everybody knows I’m their grandmother but there are times when they look and say “Oh, you look so young.” But the last couple of years, I think gravity has taken a toll. I’m looking forward to taking this journey with my daughter. I don’t want people to see only a difference when I go back to work but to say that I look well rested. They think I took a mini vacation and that’s exactly what I want them to think.

My name is Violet. I’m here at Dr. Prasad’s office because I’m interested in having some facial rejuvenation work done. I have recently lost my job and I’m back in the market place looking for a job and it’s tough out there. I’ve had very great experience but I’m up against candidates that are a lot younger. And I really want to look my best and feel my best. I think it’s great that most of the boys want you to have 15 years or more experience but they want you to look a little bit younger. So, that’s why I’m here for one reason is that I chose Dr. Prasad because he’s not only an excellent surgeon and a super specialist, since I’m having my face and eyes done, it is so important I think that you have a surgeon that’s also an artist, someone who understand that balance of the face. We were so close. Most people think we’re sisters anyway but you know, what you want to get done, what I want to get done it’s a little bit different and it’s great that we’re using Dr. Prasad because he looks at you and knows what you want. He looks at me and knows what I want and we have similar features which is interesting. He knows exactly how to do the difference and I like that. And I’m looking forward to that and I think it’s great that we’re doing it together. I also like the fact that you are doing it because you are so talented and you can’t find a job. And when I think about people that and you come home and you feel down and looking at your face and then looking at your brain, wow, she can really bring something to this company. And you always feel come home and you always got turned down only because of only the way you look and I think that’s so unfair. I’m not doing it for that purpose, I have job and I love my job and want to keep working for another 10 years if I could but i just want to look as good as I feel. I’m the same weight as I was 40 years ago and I love to be outdoors. You know how I love gardening. You know that I have these grandchildren, we have two twin girls. I love playing with them. I want to be active with them. I think the worse is when people say, you look tired. That gets to me because you know, when you sleep and you feel great and “Oh you look so tired.” All of a sudden you feel tired.

It’s a few minutes before I go into surgery. I got to Dr. Prasad office about 9:30 and they got an IV in me and Valium. I feel relaxed. I’m looking forward towards the surgery. It feels great that they made every accommodation for me to feel relaxed and positive. This is how I look now and the next time you see me, I’ll be all bandaged up and on my way to recovery. I’m looking forward to it. It’s 10 days after my cosmetic surgery with Dr. Prasad and I’m in his office for a follow-up. I feel great. I can’t tell you how wonderful it is for the whole experience. No pain with minimal swelling. I still have a little bruising in the right side of my neck over here a little behind my ears. But I don’t have make-up on at all right now so this is great, I know if I put a little cover-up on, I’ll be able to go out. You won’t even be able to notice it. But it’s been amazing. I can’t believe it’s only 10 days after. It healed so quickly, he did such a great job. I’ve seen other photos of men and women who have had cosmetic surgery and done and their face has been puffed and swollen for a lot longer.

I’m just so shocked I never had that and I guess that’s one of the reasons why I chose Dr. Prasad because he has a technique where there’s a little swelling afterwards and that makes a great difference with the recovery. As far as the procedures i have done. I had my uppers and lowers done. I had necklift and a facelift. Exactly 6 days ago, I had my procedure. I had upper and lower lid surgery. I had a face and chin lift. Today I’m at Dr. Prasad’s office and his staff checked all the stitches. Felt like nothing. Felt a little bit of pulling, that was it. I feel really good. through the whole 6 days, I’ve had no problem. I didn’t even have to take a Tylenol except for a two days later, the way I was sleeping, I bothered my neck a little bit. Other than that, I feel great. Even bruised, I still feel terrific. I can’t tell you how wonderful it feels. And the staff and Dr. Prasad, there’s no words that I can tell that I’m 72 years old and I feel terrific. I had no pain maybe a little numbness once in a while by my ears. That’s it and it’s been 4 months. I couldn’t be any happier. I’m so pleased. Wait till you see my daughter.

It’s been 4 months since I had my cosmetic surgery. I’m extremely happy with the results. I recently seen a girlfriend of mine whom I haven’t seen in approximately 14 years. No words can express how grateful we are. It was interesting to see her right after surgery, so smoothly. We recovered so quickly and so fast without incident it was amazing. Just to see how great she looks, she always looked her age but he had a tired look about her and she looked in the mirror and didn’t feel great about it. So she looks great.

I feel really refreshed and young. It’s nice to have my jawline back as far as definition. The most important thing is to find a really good surgeon because there’s a lot of people out there. Facelift Journey Rose

The Decision

I’m Rose Scalia, I’m 63 years old, I’ll be 64 in April. I’ve worked in the real estate industry most of my life in human resources. I’m also an adjunct professor in New York University. And I did some research prior to coming to Dr. Prasad. I looked into those past facelifts.

The facelifts that I’ve found, a lot of pressure when they needed to make the sale. You never really get to meet your surgeon who is basically assigned to you on your surgery day and I knew then I’ve wanted to meet my surgeon. I wanted to know who was doing my face. Your face is your cover and that’s very important to me. So, I did some research and I found Dr. Prasad and actually through my hair dresser, a friend of mine who had her eyes done. So networking was key to me. And knowing the best who can do the best and really listen to you.

I decided, well, I’m going to come to Dr. Prasad for the consultation. So I needed to tell you about part of it. I was welcomed to an office which I just aesthetically distinguished and pleasant. And I met with Nicole, the receptionist who made me feel quite welcomed which was very important in the welcoming process. And then I was preceded with Anya who would go over the details and what was needed and how we would proceed and meet with Dr. Prasad.

Dr. Prasad came in and my first impression is a smile. A smile is so welcoming and it makes you feel warm and greeted and caring. And so I knew then that that was the first thing that I liked about Dr. Prasad. He asked me what I would like to do. And I said, well, I precede myself as having this tired look. And I actually just want to get that tired look and make myself, again, match my inner energy that I had. So what I found what, which was very important to me, is that when you have a doctor that listens to you, takes time to listen to you which is extremely important for me.

Most importantly, he provides options: maybe you do need something else, maybe you don’t need something else. But those options are important and what he sees within your face. So, given the fact that he has just wonderful platter of credentials, board certified, the artistry in him. And I’ve even know that he studied art. He can precede with style and looking at the person’s face and really doing such a unique job or work or whatever it is that makes you unique and he looks at something in your face that perhaps can be enhanced.

You know when I had the wattle and so forth, it just made me feel that the traditional approach to surgery is wonderful and I’m not negating that. But the personalization that I had and I felt whatever treatment and what I received and the education from Dr. Prasad elevated us to the next level because of his artful insight. That impressed me the most and I decided to put myself in the hands of Dr. Prasad and his team.

The Experience

My fear was not of the surgery but it was fear of the anesthesia that they would use. I did not know what they would use. And when they explained to me that it was the anesthesia or the sedation really of a colonoscopy then I understood for those of you who may have had a colonoscopy. It’s a very light sedation. They give a few pills or medication to just make you feel calmer and it works. I was just very calm. I wanted to actually take a nap and they bring to the operating room and you lie down and you’re so comfortable. They give you the warmth and everything you need and they start to tell you the process. And by that time, you’re almost going to a nap and when the sedation was applied which was extremely light, I was pleasantly pleased. I believe the surgery took possibly about 2 hours and he takes his time with the prep, the surgery and the post.

The Results

Everybody expects and the expectations are: is there great pain? What do you feel? How do you feel? I need to be bluntly honest. I woke up, I was laughing with the wonderful nurses. Jeff and Gloria who take very good care of you, are very attentive and when I woke up, I even asked was it over. That’s how much I really didn’t feel a thing. When I did feel, was a just a soreness around the ears, a soreness not even pain. If you follow Dr. Prasad’s orders and what you need to do after the surgery, you will be absolutely fine. But I walked out, I was capable of walking out of my own but he had his staff walk me out which was so kind. And then, when I went back on a Sunday, which he took his time and out his family time to remove the bandage, I was pleasantly surprise. He always mentioned that there could be bruising. There was slight bruising. The stitches were so tiny that they were behind the ear that you can’t even see them. You need to follow the directions, keep them clean. And if you follow those directions, the outcome is quite amazing.

The Pageant

Well, it’s been one year since I underwent facial rejuvenation with you Dr. Prasad and your professional team who make feel so comfortable one year ago. And I was looking tired and that’s the reason why I chose to do facial rejuvenation because I didn’t want that tired outlook especially when I knew I was retiring and close to retirement. Just four months ago, at the age of 64, I retired and I call this my rejuvenation phase. A time to go out and accomplish opportunities you would never think or have time to think when you work.

So, on April 27 of this year, I entered the Miss New York Senior America Pageant. Well, I won Miss Congeniality. My platform was very important to me. My platform was “Make your age work for you”. Overall, I am very happy with the work I had done. My jaw area especially has given me the confidence that I never had, you know, my whole life. I feel like I can hold my head up, you know, strong. I always thought this is one of the most important days in this person’s life and if it were me i would want all the attention on me so in my practice we have gone out of our way to make sure that the personal level of care is exceptional and that every person feels relaxed and feels good about being here and the passion and caring and commitment of my staff to making this an excellent experience can be has been continuously reviewed as very positively by our patients

In addition I’m very committed to ensuring a quick recovery and now what that means is doing procedures in a way so that you have as little time as necessary getting anesthesia and when you talk about anesthesia I basically do all my procedures under local anesthesia with a little bit of intravenous sedation liquid valium that goes in the IV or intravenous so you feel very comfortable and relaxed but more importantly you’re able to recover very quickly you can go back to your normal life sooner rather than later unfortunately a lot of people have general anesthesia where there’s a tube and the they have to go on a respirator they need some extra time to recover and that’s something I’d essentially eliminated not just from eyelid surgery but from facelift surgery and the other procedures that we do in our practice.

I feel a personal commitment to my patients and that commitment extends beyond the one surgery that I may do for them whether it’s removing bags from under the eyes or helping them with hooding over the eyes I’m committed to helping them look their best whether it’s surgical or non-surgical and to that I what I’ve done is I’ve developed this spa and then that’s where we’re speaking from right now we’re non-invasive skin care is provided at a very high level comparable to my surgical care we have a skincare line again with the commitment for helping you look your best regardless of the stage that you’re at whether you need surgery or you don’t do surgery but we are committed to helping you look your best so that passion and the commitment in caring for you and being your doctor through and through and not just being a technician who does a procedure I think that’s a high value and is something you should look for when you’re choosing a surgeon.

Dr. Prasad: Lane artistry is such an important aspect when it comes to choosing a cosmetic surgeon I could tell you that having been an artist and being honored as an artist that it’s a real gift that either you have or you don’t and it certainly sounds self-serving, but I have trained and taught hundreds and hundreds of doctors, and I can say from experience that judgment and perception of beauty and balance is something that is intrinsic it’s not something that is in a protocol medicine tends to attract a lot of people who have called square personalities, where they’re all about order and discipline and steps but artistry is about a sense of balance and dimension, and so when I look at many of my journals we’re colleagues right about procedures have done, and they show off about and I look at them when i see well something is really off about the way people are seeing their patients and if you really want to get a sense of this you can go to the upper east side of Manhattan or to some of the more fluid areas along Island and see a lot of the quote bad plastic surgery.

So when it comes to choosing a doctor you need to just get a sense of how artistic are they and if you look at their work and probably the best way to judge in my opinion is does the do the head of patients do this it does everyone look natural and if you look natural then you’re really as an artist you’re retaining the character and the character of the face is so important to retain and as strange as it may sound that looking natural and looking like you didn’t have something done require skill it actually takes a lot of skill and a lot of judgment it’s not how much you’re doing but it’s also how much you don’t do or how you do the techniques. You need to restore the character of a person’s face then has been affected by aging changes so for example when lower eyelids are sagging you don’t want to pull them up super tight you don’t want to take away too much skin you want to create beautiful almond-shaped eyes if this person had almond-shaped before and even if they want to slightly enhanced that is artistic judgment so artistry is very important and I’m hoping that this is a useful guide for you to make your decision.

Choosing a Cosmetic Surgeon: Expertise

When it comes to expertise I have a very straightforward philosophy I believe in super specialization providing the best care possible and having the expertise to be able to do that now what does that mean in my background. I first trained in I surgery after that I trained in plastic surgery of the eyelids and the face so that field is called oculofacial plastic surgery. Many people are not aware of this and they look for a general plastic surgeon for eyelid surgery and although general plastic surgeons to get exposure for with eyelid surgery many don’t get even close to that level of experience when it comes to understanding a lot of the complexities of either surgery the reality is that when it comes to problems or anything that would happen that is unexpected all doctors in every specialty that does surgery of the eyelids will always look for an oculoplastic or oculofacial plastic surgeon to assist them to help them with their problems.

So in my opinion being a super specialist is the type of doctor that other doctors would go to and certainly my practice and doing eyelid surgery for 20 years. I’ve definitely seen that we have had many people who are physicians physician spouses physicians relatives when they do their research and they look for the best doctor for an eyelid procedure they look specifically for the most X the highest level of expertise. In addition to that I actually focus my practice on cosmetic eyelid surgery and although there are many complicated procedures related to reconstructive eyelid surgery a lot of them get incorporated into my expertise and my experience with cosmetic eyelid surgery. So there is if there are many ways to look at this and one way that I like to explain to people is that many plastic surgeons can do good eyelid surgery but very few can do exceptional eyelid surgery.

Choosing a Cosmetic Surgeon: Experience

So does experience matter when it comes to eyelid surgery well having performed either surgery for over 20 years. I could tell you it absolutely does no young surgeon wants to hear that and certainly surgeons have no shortage of ego being exposed to something having a few doing a few cases is one thing but I’m speaking to you from my operating room which I built after so many years of experience and I can tell you that there are in many fields including the music field or being a musician where you reach different levels of ability and dimension of feel when it comes to surgery and Malcolm Gladwell for example talks about the magic 10,000 hours that it took bands like The Beatles before they became exceptional and I have to admit that now that I’ve been doing this for this long.

That it is it is really true experience management experience in the area that you’re working in not just letting time pass by so at this stage of my career. I have done so much eyelid surgery and have dealt with so many issues and have a have a level and depth of knowledge and understanding that frankly they’re times I don’t even know why I know something but I know I know it and I know exactly what to do an example that I like to use is the pilot who landed that plane once it took off F. Kennedy airport and got some birds into his engine and had to be at the land in the Hudson River thanks to his years of experience as a pilot he knew exactly what to do he did in a calm way and he did what needed to be done so in so many ways performing surgery is like flying a plane or being a musician and having the experience and doing something in many ways over and over you develop a depth of confidence and knowledge and ability such that whatever you’re doing you’ve done it before and you’ve done it well and you’ve succeeded doing any surgery and in particular eyelid surgery experience is critically important so how many years have you been doing this and how many cases a week do you do what are the quality of your results can I see examples of your results and your results not just some generic format you.
